Our Care Management services are available to you remotely via phone call or telemedicine via EVisit, so you have easy and convenient access to our service and team.

Each of our Care Managers provides different services to assist you with your health needs. Below are some examples of how our Care Managers can support you.
Nurses
- One on one support to help you better understand your health conditions and concerns.
- Help you create a plan to reach your health goals and provide support along the way to overcome barriers and celebrate successes.
- Assist with creating healthy habits to improve quality of life.
- Teach you skills to better self-manage your health conditions.
Pharmacist
- Help you understand the purposes of your medications and how to take them as prescribed.
- Complete an in-depth review of your medications to ensure your medications are properly prescribed.
- Answer questions or concerns related to your prescribed and over the counter medications.
Dietitians
- Provide diet tips, meal ideas, and grocery lists to help manage your conditions.
- Support you with your health goals and create a plan to address your health and dietary needs and provide ongoing support for you to reach your goal.
- Group Classes: Our dietitians offer Diabetes and Pediatric Group Classes to help manage diabetes, reduce complications, and build healthy habits in a group setting

- Your physician may participate in our Collaborative Care Management service which allows access to additional behavioral health support if you have ADHD, Anxiety, Depression, or require Perinatal Care.
